At Treehouse the youth we serve have been impacted by the foster care system. In 2012 we began using the Check & Connect program model, in coordination with Person-Centered Planning, to impact a growth in graduation rates for this targeted population of youth. We will share how five years of data collection, program evaluation, and reporting have led to the implemention of a continuous quality improvement process that informs our internal and external stakeholders of the impact our program has for youth in foster care and has led to expansion of our work across Washington State.
